Hereditary exclusions vary wildly

Lemonade excludes hereditary conditions entirely. Trupanion, Figo, and Embrace cover them if not pre-existing. For Labs (hips), Cavaliers (heart), Bulldogs (breathing) — this is the entire ballgame. Breed matters.

The deductible type matters more than the amount

An annual deductible resets every year. A per-condition deductible (Trupanion's model) is paid once per condition — ever. For a dog with chronic allergies, the per-condition model can save thousands over a lifetime.

As early as possible — ideally before your pet's first vet visit beyond puppy/kitten vaccinations. Any condition that appears in vet records before you enroll can be classified as pre-existing and excluded. A healthy 8-week-old puppy has zero pre-existing conditions. A 3-year-old dog with one ear infection on record may have "ear conditions" excluded. The window closes with every vet visit.

On average, no — most people pay more in premiums than they collect in claims, which is how insurance works. It's worth it for: peace of mind (no euthanasia decisions driven by cost), breeds prone to expensive hereditary conditions, and protection against catastrophic bills ($5K–$20K+ for cancer, orthopedic surgery, or emergency care). If you have $10K in savings earmarked for pet emergencies, you may not need it. If you don't, you probably do.

Actual cost: the insurer pays a percentage of what your vet charged. Benefit schedule: the insurer pays a set amount per procedure regardless of what your vet charged. Actual cost (used by all plans in our recommendations) is significantly better — if your vet charges $3,000 for surgery, 90% actual cost = $2,700 back. A benefit schedule might pay a fixed $800 for the same surgery regardless of real cost.

Yes, for all plans in our recommendations. Unlike human health insurance, pet insurance in the US doesn't use a network. You can visit any licensed veterinarian, specialist, or emergency clinic. You pay the bill, submit the claim, and get reimbursed (or in Trupanion's case, they pay the vet directly at the time of visit).

Pre-existing conditions (universally excluded). Breeding, pregnancy, and whelping costs. Cosmetic procedures (ear cropping, tail docking). Elective procedures. Food and supplements (even prescription). Some plans also exclude: exam fees, wellness/preventive care (unless add-on), dental cleanings (vs. dental illness), behavioral therapy, and conditions that appeared during the waiting period.
